Kindergarten Registration for 2013-2014 School Year
Parent Meeting

(can turn in paperwork starting at 6:00, meeting will start at 6:30)

Please bring the following with you:
*Copy of Birth Certificate
*Copy of Immunization Card
*Completed Information Packet
*Completed Dismissal Plan

Student Sessions for Kindergarten Roundup will be held on Fri., April 19
(no school for current kindergarten students)
Sign up on Parent Night for your choice of day and time for your child's session.

(Iowa Law states that a child must be five on or before September 15, 2013 to be eligible for Kindergarten)

JUMPSTART

JumpStart is a class for young five-year-olds who are old enough for kindergarten, but may benefit from another year of growing before kindergarten. JumpStart students will attend kindergarten the following school year.

During Kindergarten Round-Up students may be recommended for JumpStart by their parents, preschool teacher, area education agency personnel or physicians.

IMMUNIZATIONS

All incoming kindergarten students are required to have proof of immunization turned into the school office prior to the first day of school. According to Public Health [641] Iowa Administrative Code, section 641-7.5(139A) Proof of Immunization 7.5(1) applicants, or their parents or guardians, shall submit a valid Iowa Department of Public Health Certficate of immunization to the admitting official of the school or licensed child care center in which the applicant wishes to enroll. 641-7.9(139A)Compliance. Applicants not presenting proper evidence of immunization, or exemption, are not entitled to enrollment in a licensed child-care center or elementary or secondary school under the provisions of Iowa Code section 139A.9.

PHYSICALS

In addition, all incoming kindergarten students are required to have a physical completed by a physician, registered nurse practitioner, or physician's assistant prior to the first day of class. DENTAL CHECK UP / SCREENINGS

Children entering Kindergarten must provide the school with proof of a dental check up or screening. There is an Iowa certificate of dental screening that must be completed by the dentist or dental hygienist who performed the check up or screening. This certificate needs to be turned into the school office within 4 months of the start of the school year. Please click here for the Dental Certificate form (pdf file).
